Members need to be 16 years or older to participate unless accompanied by a parent or guardian. Minors must have a parent or guardian accept the terms of this waiver/release form on his or her behalf. I understand that training for and participating in a road race is a potentially hazardous activity which could cause injury or death. I understand that I should not participate and shall not participate in Shoreline Squad and run the Shoreline Classic unless I am medically able and properly trained. I agree to abide by any decision of a race official relative to any aspect of my participation in this event, including the right of any official to deny or suspend my participation for any reason whatsoever. I assume all risks associated with running this event, including but not limited to: falls, contact with other participants, effects of the weather, including high heat and/or humidity, traffic and the conditions of the road, all such risks being known and appreciated by me. Having read this waiver and knowing these facts and in consideration of acceptance of my entry, I, for myself and anyone entitled to act on my behalf, waive and release any and all rights for claims and/or damages I may have against HSHS St. Mary's Hospital, the Decatur Running Club, all sponsors, volunteers and all organizations, and their officers, directors, agents and employees involved in Shoreline Squad and the Shoreline Classic for any injuries and/or damages suffered by me as a result of this activity. I hereby consent to receive emergency medical treatment which may be deemed advisable in the event of injury, accident or illness during my participating in the Shoreline Squad/Shoreline Classic. No warranty as to the quality of medical care is made. I also acknowledge and understand the no refund policy for the Shoreline Squad Training Program and the Shoreline Classic. I grant full permission to use photos and records of this event. By my signature, I certify that I am medically able to perform this event. By signing this waiver, I also agree this explicitly and simultaneously signs for the Decatur Running Club waiver for which I am joining as part of the squad. I agree to all of the aforesaid and certify that I understand its contents and acknowledge receipt of this waiver.
