I acknowledge that participating in the Multi Sport Extravaganza involves risk of personal injury to me and my property, and I knowingly and voluntarily agree to the terms and conditions outlined in this CONSENT, WAIVER, AND RELEASE FROM LIABILITY.

I realize and acknowledge that the Multi Sport Extravaganza (MSE) is comprised of multiple sports that can be dangerous and involve risk. These events will take place on varied surfaces, including but not limited to rugged, narrow, unpaved, rocky, steep and twisting terrain, paved roads, grass, and in and around water. I realize that as with all athletic events, there is the risk of dehydration and other medical risks.
The costs of any medical/rescue services will be my responsibility. With full knowledge of all the above facts and warnings, I intend to participate in and/or allow my child/children, ward or wards to participate in this event. I assume full responsibility for all risks associated with my participation in the event and the risk of injury caused by the condition of my property, facilities, or equipment used during the event, which may not be foreseeable by anyone at any time. I assume responsibility for my child/children, ward or wards and all of my and/or their acts or omissions. I thereby release, waive, discharge, and agree not to sue Wildlife Prairie Park, On the Go Catering, Multi Sport Extravaganza organizers & volunteers, Bushwhacker, Running Central, Shazam Racing, Little Ades, Russells Cycle & Fitness, Riverplex, Illinois Cycle & Fitness, Essential Wellness, Illinois Valley Striders, Illinois River Valley Orienteering Club, Illinois Valley Wheelm’n, Peoria Triathlon Club, Peoria Bicycle Club, Peoria Area Mountain Bike Association, Mackinaw Canoe Club, Pimiteoui Trail Association, Crossfit, Peoria Frisbee Golf Club, CEFCU, South Side Mission, Eastside Wellness Chiropractic, and other race sponsors, along with their parent companies, affiliates, their successors and assigns (Companies), their respective employees, shareholders, members, officers, directors or volunteers for any injuries, losses, damages, liabilities or expenses that are caused or alleged to be caused by their negligent acts or omissions, or the condition of the property, facilities or equipment used for the event. I agree to indemnify, defend, and hold harmless Companies and their employees, shareholders, members, officers, directors, agents, and volunteers from and against any claims, causes of action, damages, judgments, liabilities, fees (including attorney’s fees), costs and expenses incurred by Companies as a result of my unlawful actions or failures to act during the MSE. I agree to abide by all the rules and decisions of the MSE Directors.

I grant the release and full use of my name(s) and/or images and those of my children, ward or wards by Wildlife Prairie Park, the Event Director and affiliated sponsors.

EQUIPMENT – I accept for use, as is, the equipment provided to me and accept full responsibility for its care while it is in my possession. I have made no misrepresentation regarding my name, address or age.
I agree to hold harmless the indemnify Multi Sport Extravaganza, including but not limited to, its owners, agents and volunteers for any loss or damage, including any that results from claims for personal injury or property damage related to the use of this equipment, except reasonable wear and tear. I understand and am aware that bicycling and water sports are hazardous activities. I understand that the above activities involve risk of injury to any and all parts of my body. I hereby agree to freely and expressly assume and accept any and all risks of injury or death from the use of this equipment while participating in this activity.

I realize that it is mandatory that I wear a helmet and protective equipment at all times while bicycling. I understand that the helmet provided is necessary equipment and I agree to wear it. I also understand that there will be a charge for any protective equipment not returned. I further agree and understand that these items do NOT ELIMINATE THE RISK of injuries to the user’s body.

I therefore release all entities associated with the Multi Sport Extravaganza, including but not limited to, its directors, agents and volunteers FROM ANY AND ALL LIABILITY FOR DAMAGES AND PERSONAL INJURY TO MYSELF OR ANY PERSON OR PROPERTY RESULTING FROM NEGLIGENCE IN SELECTION, ADJUSTMENT, INSTRUCTION OF USE, AND/OR ANY MAINTENANCE OF THIS EQUIPMENT, accepting myself the full responsibility for any and all damages or injury of any kind which may result.

I agree that there have been no warranties, expressed or implied, which have been made to me which extend beyond the description of equipment listed on this form. I, the undersigned, acknowledge that I have carefully read this agreement and release liability and understand its contents. I understand that my signature below expressly waives any right I have to bring claim against or sue the Multi Sport Extravaganza, including but not limited to, its owners, agents or volunteers for injuries and damages.
