Waiver/Release- Must Be Read and Signed Before Mailing:
In consideration of acceptance of my entry, I, the undersigned participant, and if less than 18 years of age, my parent or guardian, intending to be legally bound, do hereby ourselves, and heirs, executors, administrators and assigns, forever waive, release and discharge any and all rights, claims and actions for damages that we or any of us may have, or that may hereafter accrue to us or any of us, against and all persons, organizations and entities associated with the event, including but not limited to Rockwood R-VI School District including sponsors, organizers, together with its directors, officers, employees and agents, any and all sponsors of the event, volunteers, and individual Race organizers, arising out of or in connection with my entry in, travel to, participating in, and returning from the Summit High School Homecoming 5K and 1 mile run/walk: and I further agree that if, despite this release and waiver of liability agreement I, the Participant or anyone on behalf of the Participant if under 18 years of age, or myself, makes a claim released in this agreement, I will indemnify and hold harmless each entitity and person released herein from any and all litigation expenses, attorney fees, loss, liability, damage, or cost they may incur as the result of such claim. I attest and verify that I am physically fit and sufficiently trained to participate in this event and that I understand that I am responsible for my own safety while participating in this event, and the post-race activities. I assume all risks associated with running in this race including, but not limited to, falls, contact with other participants, the effects of weather, including high heat and/or humidity, the conditions of the road, traffic on the course, all such risks being known and appreciated by me. I also give permission for the free use of my name and/or pictures in broadcasts, telecasts, newspapers, posters, advertising, etc for any future event given by one of the organizations and entities associated with the event, including but not limited to Rockwood R-VI School District. and any and all sponsors of the event. I understand that the entry fees are nonre- fundable.
