PLEASE READ CAREFULLY BEFORE SIGNING. THIS IS A RELEASE OF LIABILITY AND
WAIVER OF CERTAIN LEGAL RIGHTS.
In consideration of being permitted to participate in the Run for HIM 5K (the “Event”), I, the
undersigned participant (or parent/guardian if under 18), agree to the following:
1. Assumption of Risk
I understand that participating in a 5K run/walk involves inherent risks including, but not limited
to, falls, contact with other participants, weather conditions, traffic, and road conditions. I
voluntarily assume all risks, known and unknown, associated with participation in this Event.
2. Release and Waiver
I, for myself, my heirs, executors, administrators, and assigns, hereby release, discharge, and
hold harmless [Organizer Name], its officers, directors, employees, volunteers, sponsors, and
agents from any and all claims, liability, damages, or expenses arising out of or in connection
with my participation in the Event, whether caused by negligence or otherwise.
3. Medical Consent
I certify that I am physically fit and sufficiently trained for participation in the Event. I consent to
receive medical treatment that may be deemed advisable in the event of injury, accident, and/or
illness during the Event. I understand that I am solely responsible for any medical costs
incurred.
4. Photography & Media Release
I grant [Organizer Name] and its representatives the right to take photographs, video, and other
media during the Event and to use such media for promotional purposes without compensation.
5. Compliance with Rules
I agree to follow all Event rules and instructions provided by Event organizers and volunteers. I
understand that failure to comply may result in removal from the Event.
6. Minor Participants

If signing on behalf of a minor, I certify that I am the parent or legal guardian of the participant
and that I have the authority to sign this agreement on their behalf. I accept full responsibility for
their participation in the Event.
