Participation Waiver- Please read and sign below.

Please read this form carefully and be aware that in signing up and participating in this event, you will be
expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss
which you or your minor child/ward might sustain as a result of participating in any and all activities connected with
and associated with this event. I recognize and acknowledge that there are certain risks of physical injury to
participants in this event, and I voluntarily agree to assume the full risk of any injuries, damages or loss, regardless
of severity that my minor child/ward or I may sustain as a result of participating in any and all activities connected
with or associated with this activity. I further agree to waive and relinquish all claims I or my minor child/ ward may
have (or accrue to me or my child/ward) as a result of participating in this activity against the Portage Animal
Protective League and Rootstown Township/Gracie Fields, including its respective officials, agents, volunteers, and
employees (hereinafter collectively referred to as “parties”.) I do hereby fully release and forever discharge the
parties from any and all claims for injuries, damages or loss that my minor child/ward or I may have or which may
accrue to me or my minor child/ward and arising out of, connected with, or in any way associated with this activity.
I further agree that this agreement shall be governed by the State of Ohio. I have read and fully understand the
above important information, warning of risk, assumption of risk and waiver and release of all claims. If registering
on-line, my on-line or facsimile signature shall substitute for and have the same legal effect as an original form
signature.

I may be present with one or more of my pets, and/or my children. I represent that any animal brought to the
race is fully vaccinated for rabies and other communicable diseases, and does not have a history of aggressive
conduct with people or other animals. I agree to keep my animals on a leash or otherwise fully physically restrained
and under control at all times.

I understand that this is a fundraising event for the Portage APL, and that there will be no refund of any donation
or fee for weather or any other reason.

I understand that the Portage APL cannot guarantee the health or behavior of the other animals participating in
this event, or the behavior of the other participants. I also understand the health and safety risks inherent in
outdoor activities, the possibility of inc inclement weather, and the hazards at or near public roads. Therefore, in
consideration for being provided the opportunity to participate, I agree that I will hold harmless and indemnify the
Portage APL, its directors, agents, volunteers and employees, from any and all losses, claims, actions or proceedings
of every kind and character, including claims for negligence and for damages of any kind, including animal bites,
damage to property, personal injury or death for myself or my children which may arise directly or indirectly from
my presence on the above property or my participation in said activities.

This Agreement constitutes the entire understanding between the parties. This Agreement may not be amended
or modified without a writing signed by both parties.
