I, on behalf of myself and my minor child(ren) participating in the Sponsors’ (as defined below) events, if applicable (my “Minor Child(ren)”), as the case may be, acknowledge that running and biking are potentially hazardous activities. We should not enter and run or bike unless we are medically able and properly trained. We agree to abide by any decision of a race official relative to our ability to safely complete the run or bike. I, on behalf of myself and my Minor Child(ren), if applicable, hereby certify that we are in good health and have trained to run or bike the distance of the race, which we are entering.
I, on behalf of myself and my Minor Child(ren), if applicable, assume all risks associated with running and biking in these events including, but not limited to: falls, contact with other participants, the effects of weather, including high heat and/or humidity, traffic and the conditions of the road, all such risks being known and appreciated by me. I, on behalf of myself and my Minor Child(ren), if applicable, agree that my Minor Child(ren) will wear a helmet while participating in any bike event, and failure to do so will make my Minor Child(ren) ineligible to participate in such bike event.
Having read this waiver and knowing these facts and in consideration of your accepting our entries into these running or biking races, I, for myself, anyone entitled to act on my behalf, and my Minor Child(ren), if applicable, waive and release The Cure Starts Now and HP Blast Corporation d/b/a the Hyde Park Blast (collectively the "Sponsors"), their officers, directors, agents, volunteers and employees, all states, cities, countries or other governmental bodies or locations in which events or segments of events are held, all sponsors, their representatives and successors, from all claims or liabilities of any kind arising out of our participation in these events even though that liability may arise out of negligence or carelessness on the part of the persons named in this waiver. I, on behalf of my Minor Child(ren), if applicable, and for myself, am making a conscious choice to accept the consequences of the Sponsors’ negligence and understand that the Sponsors are hereby relieved from their own negligence. I, on behalf of my Minor Child(ren), if applicable, and for myself, agree that I will never prosecute or in any way aid in the prosecution of any demand, claim, or lawsuit against the Sponsors, for any loss, damage, or injury to my or my Minor Child(ren)’s person or property that may occur for any reason as a result of my Minor Child(ren)’s participation in these events. I, on behalf of my Minor Child(ren), if applicable, and for myself, accept the full cost for any such loss, damage, or injury and agree not to seek remuneration from the Sponsors. I, on behalf of my Minor Child(ren), if applicable, and for myself, understand that my Minor Child(ren)’s participation in these events involves risk and I, on behalf of my Minor Child(ren), if applicable, and for myself, voluntarily, knowingly, and without duress, choose to accept that risk
I, on behalf of myself and my Minor Child(ren), if applicable, grant permission to all of the foregoing to use any photographs, motion pictures, recordings, or any other record of this event for any legitimate purpose. We understand that bicycles, skateboards, baby joggers, roller skates or inline skates and animals are not allowed in the run events and that only bicycles are allowed in the bike events, and we will abide by these guidelines. We are aware that the Sponsors strongly discourage the use of personal audio devices while participating in this event.
I, on behalf of myself and my Minor Child(ren), if applicable, authorize any healthcare provider to release any and all information pertaining to our healthcare, medical condition and medical treatment as a result of our participation in these events to the Sponsors and their respective staffs.
