Does not Apply to Sponsors of Riders

In consideration of participation in the Social Justice Bike Ride event organized by Amachi Pittsburgh, a Pennsylvania Nonprofit Corporation, and the sponsors and partners, their agents,
owners, officers, volunteers, participants, employees, and all other persons or entities acting in any capacity related to the Social Justice Bike Ride event (collectively “Amachi Pgh”), I hereby
enter into this Amachi Pgh Waiver (“Waiver”) and agree to the following on behalf of myself, my children, my parents, my heirs, assigns, personal representative and estate:

1. That participation in this activity is purely voluntary and that I have no medical or physical conditions which could interfere with my safety in this activity, or else I am willing to assume –
and bear the costs of – all risks that may be created, directly or indirectly, by any such condition.

2. That I understand that participating in Amachi Pgh entails known and unanticipated risks which could result in physical or emotional injury, sickness, paralysis, death, or damage to
myself, to property, or to third parties. I understand that such risks simply cannot be eliminated without jeopardizing the essential qualities of participating in Amachi Pgh and accept and
assume all of the risks existing in this participation.

3. That I am highly encouraged to wear a helmet while participating in the Amachi Pgh event and will ensure that any people under the age of twelve (12) participating in the event with me will wear a helmet, as required by Pennsylvania law.

4. That any Youth (under 18) that I have registered in this event must ride in the company of a parent or guardian at all times.

5. That Amachi Pittsburgh may use any photos, videos, or likeness captured by event photographers to publicize the Social Justice Bike Ride or Amachi Pgh.

6. That I acknowledge the contagious nature of COVID-19 and other communicable viruses that could be transmitted in crowds such as those that may be found at the Amachi Pgh event and
voluntarily assume the risk that I may be exposed to or infected by COVID-19 or other viruses by participating in the Social Justice Bike Ride and that the risk of becoming exposed to or infected at may result from the actions, omissions, or negligence of myself and others, including Amachi Pgh.

7. That I agree to follow all local and state guidelines regarding masks, physical distancing, and other directives from event staff and signage to mitigate the spread of Covid-19 at the in-person
Rest Stops provided on October 2nd.

8. That I have adequate insurance to cover any injury or damage I may cause or suffer while participating in Amachi Pgh or else I agree to bear the costs of such injury or damage.
I voluntarily agree to all of foregoing and accept sole responsibility for any injury to myself or others (including, but not limited to, personal injury, disability, and death), illness, damage, loss,
claim, liability, or expense, of any kind, that I may experience or incur in connection with my participation in Amachi Pgh (“Claims”). I hereby release, covenant not to sue, discharge, and
hold harmless Amachi Pgh of and from the Claims, including all liabilities, claims, lawsuits, judgments, losses, actions, damages, costs or expenses of any kind arising out of or relating to
Amachi Pgh. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Amachi Pgh.

This Waiver will bind and be enforceable against me
and all of my personal representatives. I agree that this Waiver should be enforceable to the fullest extent of the law, and if any portion is held invalid, the remainder should continue in full
legal force and effect.

Furthermore, I agree that any Claims related to this Waiver or participation in the Amachi Pgh event shall be settled in the courts located in Allegheny County, Pennsylvania, and should
Amachi Pgh or anyone acting on their behalf, be required to incur attorney’s fees and costs to enforce this Waiver, I agree to indemnify and hold them harmless for all such fees and costs.
By purchasing a ticket to the Social Justice Bike Ride, I acknowledge that I have had sufficient opportunity to read this Waiver; have read and understood it; and that I agree to be bound by its terms by checking the box below.
