Angie Barry Memorial 5K Race/Walk

I understand and acknowledge that my participation in the Angie Barry Memorial 5K Run/Walk involves a risk of injury, including bodily injury, and possible death and assume the risk for same.  I also understand and acknowledge that I should not enter and participate unless I am medically able and properly prepared.  On my own behalf and on behalf of my heirs and legal representatives and to the fullest extent permitted by law, I hereby release and discharge the Angie Barry Memorial 5K Run/Walk Committee, the Topeka West Track and Cross Country Programs, Hummer Sports Park,  and their respective members, representatives, directors, volunteers, coaches, and sponsors of and from any and all liability for injury, death, or damages and/or any other claims, demands, or losses incurred by me in connection with any aspect of the 5K run/walk.  My signature acknowledges that I have read the above waiver and I agree and accept all terms and conditions set forth herein.
