By submitting this entry I, intending to be legally bound for myself, my heirs, executors, and administrators waive, release and forever discharge any and all rights and claims for damages I have or may have against the Dysart Unified School District, Raceplace Event Systems Inc, the City of El Mirage, the organizer of this event, its principals, its employees, its volunteers and, all sponsors and their representatives for any and all claims, damages, demands or actions whatsoever in any manner, as a result of my participation in the event registered for, including travel to and from the event. I hereby consent to medical treatment in the event of injury, accident, and/or illness during the event. I hereby grant full permission to any and all of the foregoing to use my name and likeness in any broadcast, telecast, video, or print media reporting or advertising or advertising of the event.

In Consideration of being able to participate in the Dysart Dash, I agree to assume all risks inherent in participation in such program, whether they are apparent to me or not.  I certify that I am in good physical health and fit to participate.  Nevertheless, I acknowledge that participation carries an inherent risk of injury to my person and damage to my property.  I hereby waive and release, for myself and for my heirs and assigns, any and all claims, causes of action, or liabilites which may hereafter accrue aginst the City of El Mirage, and its affiliates, their agents, employees, volunteers, officers, directors, succesors and assigns, and any and all sponsors, their representatives and succesors, that may arise as a result of my participation in the Dysart Dash, including any and all claims for personal injuries caused by the City of El Mirage's negligence.  Further, I hereby grant full permission to any and all of the foregoing to use any photographs, motion pictures, recordings, or any other record of these events for any legitimate purpose, including commercial advertising, without monetary payment to me.  (This information is protected by the Privacy Act.)
