Fort Riley Bow Slinger 3-D Archery Tournament

Liability Release (must be signed). I intending to be legally bound, do hereby for myself, my heirs, executers, and assigns, waive, release and forever discharge any and all rights and claims for damages that I may have or which my hereinafter accrue to me against the United States Army, Fort Riley U.S. Army Garrison, Fort Riley Directorate of Family, Morale, Welfare and Recreation, its agents, officers, employees, sponsors, their representatives or successors arising out of my association with or participation in the Fort Riley Outdoor Adventure & Travel Center Archery Range. I assume all risk associated with this activity including, but not limited to, contact with other participants, the effects of the weather, and the danger of injury or even death, all such risks being known are appreciated by me. I hear by consent to emergency treatment in the event of my injury or illness.

Furthermore, I agree to reimburse the provider, as required for any cost, debts, claims or liabilities predicated upon the loss of, damage to, or destruction of any property owned by the provider which occurs from my improper or negligent use of, or involvement with the provider facilities, equipment or services.

Participants under the age of 18 must have an adult guardian sign this form and be present during the activity.
