2019 RUN FOR WALK WAIVER AND RELEASE

Please read this form carefully and be aware that by participating in the 2019 Randy Walker Memorial 5k (“the event”), held at Northwestern University on July 28, 2019, you will be waiving all claims for injuries you might sustain arising out of your participation.

As a participant in the 2019 Randy Walker Memorial 5k on July 28, 2019, I recognize and acknowledge that there are certain risks of physical injury and I agree to assume the full risk of any injuries, damages, or loss that I may sustain. Such risks are inherent to this event may include: bodily injury. I do hereby fully release and discharge Northwestern University, its trustees, officers, agents and employees from any and all claims from injuries, damages or loss that may accrue to me on account of my participation in the 2019 Randy Walker Memorial 5k. I further agree to indemnify and hold harmless and defend Northwestern University, its officers, agents and employees from injuries, damages and loss sustained by me and arising out of, connected with, or in any way associated with my participation in the 2019 Randy Walker Memorial 5k. I have fully read and understand the foregoing. It is my express intent that this agreement shall bind the members of my family, my heirs and assigns. This agreement shall be construed in accordance with the laws of the State of Illinois.

I have read and fully understand the above waiver and release of all claims.

2019 RUN FOR WALK MINOR WAIVER AND RELEASE ADDENDUM

Please read this form carefully and be aware that by having your child participate in the 2019 Randy Walker Memorial 5k (“the event”), held at Northwestern University on July 28, 2019, you will be waiving all claims for injuries your child might sustain arising out of his/her participation.

I recognize and acknowledge that there are certain risks of physical injury to participant in the event, and I voluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child/ward may sustain as a result of said participation. I further agree to waive and relinquish all claims against Northwestern University, its officials, agents, volunteers and employees that I or my minor child/ward may have (or may accrue to me or my minor child/ward) as a result of participating in the event.

Further, I do hereby fully release and forever discharge Northwestern University and the City of Evanston from any and all claims for injuries, damages or loss that my minor child/ward or I may have or which may accrue to me or my minor child/ward and arising out of, connected with, or in any way associated my child’s participation in the event.

I have read and fully understand the above waiver and release of all claims.
