WAIVER: In consideration of accepting my entry, I hereby agree to participate in this event at my sole risk and I assume any and all risks which may be associated with this event.  I hereby waive and release any and all rights and claims of any nature which I may have or claim against the organizers and all others connected with this event, including without limitation, Five Acres Animal Shelter and its agents, representatives, employees, contractors, sponsors, officers, directors, successors and assigns (collectively, "Releasees"), for any and all liability and/or damages of any kind whatsoever suffered by me or my pet as a result of taking part in this event or any related activities, including without limitation, any and all damages (i) to property and/or pets, and (ii) for personal injury.  I hereby represent and certify that my pet is in good health and up to date on immunizations, and that my pet and I are physically fit to participate in the walk mentioned above.  In addition to the foregoing, I hereby consent to and authorize multiple use by Releasees, or any of them, or any one authorized by Releasees or any of them, of any and all photographs, videos, any other likeness (hereinafter, the "Images") Releasees, or any of them, have as of this date taken of me, and any reproduction of them, and all media, whether now known or hereafter invented, for any purpose whatsoever, including without limitation, the sale, internet transmission, publication, broadcast, display, and exhibition thereof in any electronic or print media for any purpose, including without limitation, in commercials, promotions, grant applications, advertising, trade, and by any and all means, methods or processes, whether now known or hereafter invented, without notice to me and without any compensation to me.
