Appropriate apparel and footwear are required to participate in all Employee Wellness programs. Dress shoes are not allowed. Employee Wellness reserves the right to cancel, reschedule, or move classes and/or programs to alternate locations. Employee Wellness also reserves the right to change, replace, or find substitutes for all instructors. Children are prohibited from participation in all Fitness Program classes and may not be left unattended. Please be aware of all UNM parking regulation and fees. You may call Parking and Transportation Services at 277-1938 for more parking information. In consideration of the acceptance for my application in the Employee Wellness programs, I hereby waive, release and discharge any and all claim for death, personal injury, or property damage which may occur to me as a result of my participation in the Fitness Program. I understand the strenuous nature of the Fitness Program and accept full responsibility for any risk involved. I also accept full responsibility for my health and wellbeing while voluntarily participating in any exercise or fitness program. Future, I hereby grant full permission to any and all of the foregoing to use photography, videotapes, motion pictures, recording, or any other record of this event for any legitimate purpose.  I agree to follow all policies and understand I may lose all privileges associated with this program should I fail to comply with all of the University of New Mexico and Employee Wellness policies.
