PERMISSION FOR ENROLLMENT AND RELEASE OF KIWANIS LIABILITY: I understand that even when every reasonable precaution is taken, accidents or illness can sometimes happen. Therefore, in exchange for the Kiwanis Club allowing me, my child or other family member to participate in the Calusa BUG Chase 5K, I understand and expressly acknowledge that we do so at our own risk. I release the Kiwanis Club, the Fort Myers Track Club and the Calusa Nature Center from all liability for injury, loss or damage connected in any way whatsoever to participation in this activity. I understand that this Release includes, but is not limited to, any claims based on negligence, action or inaction of the Kiwanis Club or Fort Myers Track Club, it’s members or directors. I have read the form and grant permission for my child to participate (if applicable).  I authorize appropriate medical personnel to administer emergency medical treatment to me, my child or other family members. I also understand that I am solely responsible for all costs incurred as a result of such treatment. I have read and voluntarily signed this Authorization and Release. I grant full permission for organizers to use my name, likeness, or voice and photographs, videotapes, or quotations from me in accounts and promotions in any medium of this event, and of the activities of The Fort Myers Track Club. This permission is perpetual and worldwide.
