PARK CITY HOSPITAL 5-K9 RUN, WALK, JOG ASSUMPTION OF RISK, RELEASE OF LIABILITY & INDEMNITY AGREEMENT   PLEASE READ THIS AGREEMENT CAREFULLY AND PRINT A COPY FOR YOUR RECORDS.  BY CHECKING THE "ACCEPT" BOX, YOU AGREE TO THE TERMS OF THIS AGREEMENT AND THE ACTIVITY RULES. IF YOU DO NOT AGREE TO THE TERMS OF THIS AGREEMENT, THEN YOU MAY NOT PARTICIPATE IN THE ACTIVITY.    In consideration of participating in the Park City Hospital 5-K9 Run, Walk, Jog Activity (the “Activity”), I hereby agree with IHC Health Services, Inc., Park City Hospital, City of Park City, and Paws for Life Utah as follows.  I understand that “I,” “me,” and “my” refer to me as the participant in the activity and the person responsible for the dog(s) participating in the activity with me.  “Dog” used singularly in this agreement includes all dogs participating in the Activity with me for which I have responsibility.   I acknowledge that I am responsible for my conduct and the conduct of my dog, and my participation in the Activity may be terminated if I fail to comply with the provisions contained herein and any rules relating to the Activity.   ASSUMPTION OF RISK: I assume all risks associated with running in this event including, but not limited to: falls, tripping, contact with other participants and their dogs, effects of weather, including high heat, traffic, conditions of the road/course, all such risks being known and appreciated by me.   WAIVER, RELEASE AND INDEMNIFICATION: Having read this waiver and knowing these facts in consideration of you accepting my entry, I, for myself and anyone entitled to act on my behalf hereby agree to release, waive, covenant not to sue, indemnify, and hold harmless IHC Health Services, Inc., Park City Hospital, City of Park City, and Paws for Life Utah and their agents, officers, representatives, volunteers, employees and other affiliated parties (collectively the “Releasees”) from all liability arising out of any and all claims, demands, losses, damages, suits, actions, judgment of any kind and description which may occur to be suffered by me as the race participant by reason of activeness arising out of my participation in the Activity, even though that liability may arise out of negligence or carelessness on the part of the person named in this waiver.   I further agree to release, waive, covenant not to sue, indemnify, and hold harmless the Releasees from all liability arising out of any and all claims, demands, losses, damages, suits, actions, judgment of any kind and description which may occur, including those arising out of their negligence or carelessness, that may arise from or in connection with my dog’s participation in the Activity, including personal injuries to or death of my dog.   Dog Rules: A maximum of two dogs per registered runner. All dogs must be on a leash or otherwise tethered/contained at all times. You certify your dog is current on all shots, at least 6 months of age and of good health All male dogs must be neutered and no female dogs in heat will be allowed to participate.  You must clean up after your dog.   Your dog will NOT be allowed to participate if: You cannot certify that your dog is current on all required vaccinations. Your dog displays an aggressive or dangerous personality.   I AM AWARE THAT THIS IS AN ASSUMPTION OF RISK, WAIVER OF LIABILITY, AND INDEMNIFICATION AGREEMENT BETWEEN ME AND THE RELEASEES.  I HAVE CAREFULLY READ THIS AGREEMENT AND I AM ACKNOWLEDGING THAT I FULLY UNDERSTAND ITS CONTENT AND AGREE TO ITS TERMS BY ENTERING MY FULL NAME AND CHECKING THE “ACCEPT” BOX.
