By signature. I attest that I am physically fit and
sufficiently trained to participate in this event. I know that running a road
race is a potentially hazardous activity. I am in proper physical condition to
compete in this run/walk and assume all risks associated with my participation
including, but not limited to, falls, contact with other participants, and the
effects of the weather including high heat and/or humidity, traffic, and the
conditions of the road, as such risks being known and appreciated by me. I have
full knowledge of the risks involved. I further acknowledge that the race
course may contain uneven surfaces conditions, including but not limited to pot
holes, curbs, and sewer covers which may create a risk. I also acknowledge that
the race is on public roads and I accept these risks. I accept responsibility
for, and assuming the risk of any injury or damage and costs to my person or
dependent children which might arise directly or indirectly as a result, and or
participation in a Snap Fitness Program. as well as transportation to and from
the race. I hereby expressly release, discharge and hold harmless from any

grant the agents of this event permission to use photos or any other record of
