2018 5K Turkey Trot- Benefit Diller Blind Home

Waiver: 
 
Knowingly, and at my own risk, I, or a minor under the age of eighteen for whom I am signing, am participating in Challenged Children's Charities Corp. and the Helen L. Diller Vacation Home for Blind Children Turkey Trot on November 23, 2018. 
 
 I do hereby waive and release any and all claims against Challenged Children's Charities Corp. and the Helen L. Diller Vacation Home for Blind Children, and all event sponsors and any employee, volunteer, or officials of these organizations from any claim of injury not limited to falls, contact with other participants, the effects of the weather, (including death) that I may incur as a result of my participation in the event.  
 
  I further hereby certify that I have full knowledge of the risks involved in this event, and I am physically fit and sufficiently trained to participate. If, however, as a result of my participation in the Helen L. Diller Vacation Home for Blind Children Turkey Trot, I require medical attention, I hereby give consent to authorize medical personnel to provide such medical care as deemed necessary.  
 
  The undersigned further grants to indemnify, defend, and hold the entities named above harmless from and against any and all claims, liabilities, losses and damages, costs expenses (including attorney's fees) judgments and penalties arising out of any of my, and our said minors, acts or omissions to act. I understand that Challenged Children's Charities Corporation and the Helen L. Diller Vacation Home for Blind Children reserves the right to use any and all participant's names and/or likeness with regard to promotional and/or advertising materials. 
 
 I understand that all entry fees are non-refundable.
