I, the participant, intending to be legally bound, understand and agree that I am voluntarily participating in the WCSO Zombie Fun Run, at my own request and my own risk. I acknowledge that I am aware of all the inherent risks in this event, the risks of participating in an event with possible physical requirements and certify that I am physically fit, have not been otherwise informed by my physician and know of no restriction, disease or condition that would in any way present a risk to my health or prevent me from actively participating in this event. In consideration of being permitted to participate in the WCSO Zombie Fun Run, I assume full responsibility for any injury or loss that may occur in connection with my participation, and I, on behalf of myself, my successors in interest, heirs, assigns, ad representatives hereby full release and agree to hold harmless the WCSO, JDRF, its employees, officers, directors, and agents from any and all liability, claims, rights, or actions for death, bodily injury, property damage, or any other loss or inconvenience whatsoever, suffered by me or cause by me, at any time hereafter occurring as a result of my voluntary participation in the WCSO Zombie Fun Run. If I am under the age of 18, I understand that a parent or legal guardian must accompany me at all times and must acknowledge waiver on my behalf by providing name and email address in the field to the left. I authorize JDRF, WCSO, or anyone authorized as a representative of WCSO or JDRF to use my name and/or any photographs of me, which I have provided are taken of me at the event to promote or advertise any WCSO Zombie Fun Run. Such promotion or advertising may include but is not limited to the publication, display, and exhibition of my name and/or my photographs. I agree that any photography, negatives, and reproductions are the sole property of JDRF and WCSO with full right of disposition in any manner whatsoever. I declare my consent to be irrevocable and release JDRF and WCSO from any and all claims whatsoever in connection with the use of my name or photographs as previously described.
