I know that competing in a road race is a potentially hazardous activity. I should not enter and compete unless I am medically able and have trained to cover the distance of this event. I agree to abide by any decision of a race official relative to my ability to safely compete in this event and I assume all risks associated with participating in this event including but not limited to: falls, contact with other participants, the effects of weather, including high heat and/or humidity, traffic and the condition of the road, all such risks being known and appreciated by me. Fully understanding the nature of this activity I hereby, for myself, my executors and administrators waive and release and hold harmless the We Can Event, Columbus Community Hospital and any of their agents, employees, officers, board members, and volunteers, the City of Columbus and all Sponsors, and any of their agents, employees, officers, council members, meet officials, volunteers and workers for any and all rights and claims for damages or costs I may incur. This includes personal injury, death, or property damage suffered by me, or that I may cause to others as a result of my participation in this activity. I have read and clearly understand the above statements, and realize this is a contract between myself and Columbus Community Hospital and is a release of liability. Further, I hereby grant full permission to any and all of the foregoing to use photographs, videotapes, motion pictures, and recordings of me, or any other record of this event, for any legitimate purpose. I sign it of my own free will. If under 18, parent/guardian must sign.
