Walton County Prevention Coalition&nbsp;<br>&nbsp;<br>LakeFest 5k Run/Walk and Dash N’ Splash Fun Run, Waiver & Release Form&nbsp;<br>&nbsp;<br>I acknowledge that my participation in the LakeFest 5k Run/Walk and Dash N’ Splash Fun Run involves a risk of injury, including bodily injury, and assume the risk for same. On my own behalf and on behalf of my heirs and legal representatives and to the fullest extent permitted by law, I hereby release and discharge Chautauqua Offices of Psychotherapy and Evaluation Inc., Walton County Prevention Coalition, event sponsors, and their respective directors, officers, employees, affiliates, members, agents and representatives, of and from any and all liability for injury, death, or damages and/or any other claims, demands, losses or damages, incurred by me in connection with any aspect of and/or arising out of the 5k run and walk.&nbsp;<br>&nbsp;<br>If I am an employee of Chautauqua Offices of Psychotherapy and Evaluation Inc., I acknowledge that my participation in the LakeFest 5k Run/Walk and Dash N’ Splash Fun Run is completely voluntary and does not constitute part of my work-related duties. I understand that my decision to participate, or not participate, in this activity will not affect my job status.&nbsp;<br>&nbsp;<br>I acknowledge that the LakeFest 5k Run/Walk and Dash N’ Splash Fun Run will be held rain or shine and that in the event of dangerous weather conditions the race organizer may name an alternate date, venue, and/or route for the LakeFest 5k Run/Walk and Dash N’ Splash Fun Run event to be held. All registrations are final and no refunds will be provided.&nbsp;<br>&nbsp;<br>I acknowledge that all photographs, video or any images taken by the event organizers of me or that include my image may be used without my further permission in all manners that the event organizers deem appropriate and I hereby consent to that use.
