Waiver & Release of Claims and Permission Slip

• I know that participating in a Virginia Beach Parks & Recreation Sunday Runday Event is a potentially hazardous activity. I should not enter and participate unless I am medically able and properly trained. I agree to abide by any decision of a race official relative to my ability to safely complete the run; however, I also acknowledge that race officials are not responsible to know my medical condition or my ability to participate in and complete this event. I assume all risks associated with participating in Virginia Beach Parks & Recreation Sunday Runday activities including, but not limited to falls; contact with other participants, volunteers and spectators; the effects of the weather, including high heat and/or humidity; traffic and the conditions of the road; and holes and obstructions along the route, all such risks being known and appreciated by me. Having read this waiver and knowing these facts and in consideration of your accepting my application, I, for myself and anyone entitled to act in my behalf, waive, release and hold harmless any and all race directors, officials, volunteers and sponsors, including but not limited to Virginia Beach Parks & Recreation Foundation and the City of Virginia Beach and their representatives and successors from all claims of liabilities of any kind arising out of my participation in this event.

• In exchange for me and/or my child(ren), named in the registration, being allowed to participate in the above-referenced program, I agree to waive, release and forever discharge any and all claims, rights and causes of action against the Virginia Beach Parks & Recreation Foundation and the City of Virginia Beach and their respective officers, officials, employees, and agents for injury or damage caused or alleged to be caused as a result of my or my child(ren)’s participation in the above referenced program. I agree to this waiver and release for all my heirs, personal representatives, next of kin, and assigns. I hereby confirm that I am the parent or legal guardian of the child(ren) listed in this paragraph.

• My digital signature certifies that I grant permission for my child(ren), listed above, to participate in the Virginia Beach Parks & Recreation Sunday Runday Event that is scheduled to take place on July 23, 2017 (“Event”) and further, will assure that, if dropped off at the Event, my child(ren) will know the arrangements for getting home in a safe and timely way.

• I understand and voluntarily assume all risks associated with my and my child (ren)’s participation in the above-referenced program, including the possibility of accidental or other physical injury during the program and/or participation in programs conducted by the Virginia Beach Parks & Recreation Foundation and the City of Virginia Beach, including programs co-sponsored by other departments. This waiver and release of all claims and assumption of risk is intended to be as broad and inclusive as permitted by the laws of the Commonwealth of Virginia. If any portion of this document is held invalid, the remainder shall continue in full force and effect.

• My signature certifies that I grant the City of Virginia Beach the right to photograph my child(ren) and the right to use said photographs and information in connection with publicizing or promoting the Virginia Beach Parks & Recreation Foundation and the City of Virginia Beach, its services or departments and agencies.

I have carefully read and fully understand the meaning of this document. I agree to all the terms set forth herein, and I have digitally signed this waiver.
