Assumption of Risk, Waiver and Release, and Permission for Use of Photo/Participation in Event: I desire to enter and participate in the Blazer 5K organized by the Campus Recreation Department at the University of Alabama at Birmingham. I know that running and walking are potentially hazardous activities. I understand that I should not enter a walk or run unless I am medically able and properly trained and I accept full responsibility for making that decision. Notwithstanding that this is my responsibility; I agree to abide by any decision of a race official who requests that I do not participate due to concerns about my ability to safely complete the race. I hereby certify that I am in good health and have trained to walk or run the distance of the race I am entering. I assume all risks associated with running or walking in this race including but not limited to: soreness, falls, and contact with other participants, the effects of weather, including high heat or humidity, traffic and the conditions of the road, all such risks being known and appreciated by me. Having read this waiver and having knowingly and voluntarily assumed the risks associated with participating in the Blazer 5K, and in consideration of your accepting my entry into this race, I, for myself and anyone entitled to act on my behalf, waive and release the Board of Trustees of the University of Alabama at Birmingham and its operating division, UAB, its officers, directors, assistants, volunteers, and employees from all claims and liabilities of any kind arising out of my participation in this event even though that liability might arise out of negligence or carelessness on the part of the persons named in this assumption of risk and waiver. I grant permission to all of the foregoing to use photographs of myself, recordings including myself or any other record of my participation in this event for any legitimate purpose, either online or print including publically purposes associated with UAB Wellness programs.
