Phi Delta Theta at the University Of Southern Indiana is hosting a Phi-K (5K) Run/Walk on Saturday, August 27th, 2016. Everyone is welcome to sign up and participate. Proceeds from this event go to help eradicate Lou Gehrig's Disease (ALS), host a philanthropy/fundraiser, and help further the University of Southern Indiana community. T-shirt, water, bananas, and granola bars will be provided for the runners/walkers. The course is made up of both street and off road terrain.

I, being over the age of eighteen (18) desires to participate

in Phik Walk/Run, Saturday, August 27th, 2016 at the University of Southern Indiana.

I understand that this activity ( Phik Walk/Run ) are/or can be hazardous, and has many inherent risks, including but not limited to contusions, concussions, breaks, cuts, sprains, strains, tears, and even death. I, for myself, my heirs, successors or assigns, hereby assume any and all risks, dangers, and hazards and the possibility of personal injury, death, property damage or loss attendant to these activities and waive any claim that I might have in any way resulting from my participation in this activity. I understand that I am responsible for my own safety, health and welfare as well as any medical expenses should I be injured during this activity. It is prudent and required under the Affordable Care Act to have health insurance covering me.

I acknowledge that it is my obligation to ensure my participation is consistent with any physical limitations I may have, and to consult with my physician to ensure that I am medically able to participate in this activity.

In consideration of being afforded this opportunity to participate in Phik Walk/Run , I, for myself, my heirs, successors or assigns, hereby release and hold harmless the University of Southern Indiana, it’s Board of Trustees, agents, servants, and employees from any and all claims, demands, causes of action or damages which may accrue on account of bodily or personal injury, property damage, or death arising from the negligence of the aforesaid parties.

If any part of this Agreement is for any reason found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

I have read and I understand that this is an ASSUMPTION OF ALL RISKS AND A GENERAL WAIVER OF ANY AND ALL CLAIMS OR CAUSES OF ACTION, which I may have or might accrue as a result of the foregoing activities.
