I know that running a race is a potentially hazardous activity and I should not enter a run/walk unless I am medically able and properly trained. I agree to abide by any decision of a race official relative to my ability to complete the run/walk safely. I assume all risks associated with running in this event, including, but not limited to; falls, with other participants, the effects of weather, the conditions of the course, all such risks being known and appreciated by me. Having read this waiver and knowing these facts and in consideration of your accepting my entry, I, for myself and anyone entitled to act on my behalf, waive and release all sponsors, their representatives and successors from all claims or liabilities of any kind arising out of my participation in this event or carelessness on the part of the person named in this waiver. Further, I grant permission to all the foregoing to use photographs, motion pictures, recordings or any other records of these events for any legitimate purpose. I understand that bicycles, skateboards, baby joggers, roller skates or blades, animals and radio headsets are not allowed in the race and I will abide by these guidelines.  
 
I attest and verify that I am mentally and physically fit and able to participate in this event and acknowledge that I am aware of the inherent risks in participating in an athletic event of this type   
 
Minor Participants 
I as the parent guardian grant my/our child permission to participate in all PTA sponsored events for the school year 2015 to 2016. The undersigned parent or guardian assumes all risks in connection with the student's participation in any and all of the PTA sponsored activities. I the undersigned, intending to be legally bound, do hereby for my self and heirs, executors, administrators and assigns, forever waive, release and discharge the Maryland State PTA, all PTA officers, employees, agents, and volunteers from all liability, claims or demands for any damage, loss or injury to the student, the student's property, or parent's property or to myself in connection with participation in these activities, unless caused by the negligence of the PTA.
