In consideration of your acceptance of this entry, I, the undersigned, intending to be legally bound, hereby for myself, my heirs, executors and administrators, waive and release any and all rights and claims for damages I may have against KTVK Television; Pepsico; Blue Cross Blue Shield of Arizona; Mollen Clinic; City of Phoenix; USAT&F; Raceplace Event Systems Inc: all other sponsors; and any and all officers, partners, members, managers, affiliates, agents, contractors or employees of the foregoing for any and all injuries sustained and suffered by me during this race. I verify that I am physically fit and have sufficiently trained for competition in this event and a licensed medical doctor has verified my physical condition. If, however, as a result of my participation of this race, I require medical attention, I hereby give my consent to such authorized personnel. I also understand that in the event this race cannot be held as scheduled due to an act of God or circumstances beyond control, the race is not liable to refund any money paid by me to participate.
