In consideration of acceptance of this application to participate in The Ogden Newspapers Half Marathon Classic, The Ogden Newspapers 5K Walk and Run for Health, Ogden Mile, Ogden Fun Run and/or Ogden Tot Trot (in any event, the 'Run'), to be held in Wheeling, West Virginia, on May 28, 2016, I waive all claims for myself, my heirs, successors and assigns against the City of Wheeling, the race sponsors, promoters, volunteers, and officials, and all related parties and affiliates, due to accident, injury or illness which may result from my participation.  I attest and verify that I have full knowledge of the risk involved in these events.  I am physically fit and sufficiently trained to participate.  I have read the above statement, I understand it, and my signature confirms its full acceptance.

I also agree for the same consideration aforesaid to allow use of photographs, video tapes or other likenesses of myself taken at the Run to be used by the race sponsors or their assigns for any purpose.
