I, intending to be legally bound, do hereby, for myself, my heirs, executors, and assigns, waive, release and forever discharge any and all rights and claims for damages that I may have or which may hereinafter accrue to me against the United States Army, Fort Riley, U.S. Army Garrison, Fort Riley Directorate of Family and Moral, Welfare & Recreation, its agents, officers, employees, sponsors, their representatives or successors arising out of my association with or participating in the Fort Riley event. I know that participating is a potentially hazardous activity and that I should not enter and participate unless I am medically able and properly trained. I understand that I do not need a physical examination to participate and that I participate at my own risk. I assume all risks associated with falls, contact with other participants the effects of the weather, traffic, the conditions of the course and the danger of injury or even death, all such risks being known and appreciated by me. I hereby consent to emergency treatment in the event of my injury or illness. Information is solicited in accordance with DOD and Army privacy programs (32CFR 310; 505).  
I covenant and agree that I will never prosecute or in any way aid in the prosecution of any demand, claim, or suit against the aforementioned parties for any loss, damage or injury to my person, child or property that may occur from any cause whatsoever as a result of my actions or as a result of my participation in this activity. I also understand and agree that I may be liable for any damage or loss to the above parties that is caused by me in any manner, including but not limited to simple negligence, gross negligence, willful misconduct and fraud.  
I understand and agree that I will indemnify and hold harmless the aforementioned parties for any loss or damage to third parties that is caused by me in any manner, including but not limited to the elements set out above, as a result of my participation in the activity.  
As used herein, the term “United States Government” includes any officer, agent, agency, representative, official, or employee of the United States Government.  
I understand that participating in this event involves the risk of severe physical injury and/or death that cannot be completely eliminated  
I understand, agree and accept that some of the obstacles may go through water that has not been tested for chemicals, disease or contamination.  
I understand that the event course may contain wild/dangerous animals, insects and plants. I agree that DFMWR reserves the right to cancel the event in the event of weather (including but not limited to, heat, tornados, earthquakes, fires, storms, lightning and floods), accidents, acts of war or terrorism, military conflicts or riots or for any reason that, in DFMWR’s discretion, will protect the safety and security of event participants and spectators.  
I acknowledge that I have read and fully understand the important information above, that I have carefully analyzed, accepted, and agreed to all terms herein, that I sign on my own free act and deed and that I understand that I have given up legal rights by signing below.  
I acknowledge that this form is consideration for permission to participate in the event and my participation in the event will be denied if my signature ( or the signature of my parent or guardian if I am under the age of 18) does not appear on this form prior to my participation in the event.   
Complete section below if you are a parent or legal guardian of a participant under the age of 18 I, the parent or guardian of the above named participant (minor), give my approval for minor’s participation in the event. I assume all risks and hazards incidental to minor’s participation in the event and I release, waive, discharge, absolve, indemnify, agree to hold harmless and covenant not to sue the releasees for any claim arising out of injury to the minor and from any and all claims, causes of action, obligations, lawsuits, charges, complaints, controversies, damages, cost or expenses of any kind, nature, or description, whether direct or indirect, in law or in equity, in contract or in tort, or otherwise, whether known or unknown, arising out of or connected with minors attendance at and participation in the event, whether or not caused by the releasees’ negligence or gross negligence.
