Waiver and Release
I know that running a mud run is potentially hazardous activity. I should not enter and run unless I am medically able and properly trained.  I attest that I am physically fit for the Race, I have adequately trained for the Race, and that I am aware of the risks associated with the Race, including not by means of limitation, the difficulty of navigating the obstacles integrated into this event and have attired myself for the Race accordingly. I agree to assume all risk in connection with the Race, including not by means of limitation, my decision to enter the Race, the condition of the Race course, potential wildlife encountered on the Race course and/or the suitability of the Race course for the Race event. I hereby on behalf of myself, my heirs, executors and administrators waive and release any and all rights and claims against the following entities and persons, and agree to indemnify and hold harmless the following entities and persons: Anchorage Community Mental Health Services, Inc., Municipality of Anchorage Parks and Recreation and any other entity involved with the Race such as sponsors, their officers, directors, agents, employees and event volunteers, with respect to any and all liability of any nature in any way arising out of the Race and my participation therein, including not by means of limitation, the condition of the premises in or around the Race course. I further understand that my picture may be taken during the course of participation and/or around the festival and that my likeness may be used without further compensation in media of any form. The foregoing release of liability includes all liability with respect to my travel to or from the Race.

PART “B” - PARENT/GUARDIAN WAIVER - RELEASE FROM LIABILITY (If applicant is under 18 years of age, the parent(s) or guardian(s) must execute in addition to the above, this following waiver;) The Undersigned, referred to as the parent(s) and natural guardian(s) or legal guardian(s) of the above named player(s) thereby represent that he/she (they) is (are), in fact, acting in such capacity and agrees to save and hold harmless and indemnify each and all of the parties herein referred to above as releases from all liability, loss, cost, claim or damage whatsoever may be imposed upon said releasees because of any defect in or lack of such capacity to so act and release said releasees on behalf of the undersigned.
