The Organ Trail Run 10K and 2 Mile Walk

Event Waiver and Release of Liability: Mid-America Transplant Services and Team Transplant St. Louis is participating in the sponsorship of The “Organ” Trail Run, a sporting event, on Saturday May 23, 2015. This sporting event involves a contact sport, and it is its nature that there is certain risk of injury to the participants and the viewers. I hereby acknowledge that I have read and I am familiar with the restrictions and conditions of participating in this sporting event. I understand that Mid-America Transplant Services do not agree to my participation in this sporting event without signing this instrument. I hereby, for myself and my heirs and personal representatives, waive, release and forever discharge Mid-America Transplant Services, Team Transplant St. Louis, Big River Race Management, Castlewood State Park, the State of Missouri, and their officers, agents, participating in, or being present at, this sport event. Parent or Legal Guardian Initials required if registrant is under the age of 18 years old.
