PLEASE READ CAREFULLY BEFORE SIGNING THE WAIVER/RELEASE FORM: ROUNDUP LAKE In consideration of this entry, I understand and acknowledge that multisport events, which includes the sports of swimming, biking, and running, is an extreme test of my, or my child/minor’s, physical and mental limits and is a rigorous physical activity. I understand that these limits carry the potential for death, serious injury, impairment, and property loss and damage. I AM AWARE AND ACKNOWLEDGE AND UNDERSTAND THAT THE RISKS INVOLVED IN THIS ACTIVITY MAY APPLY TO ME OR MY CHILD/MINOR, AND HEREBY I ASSUME THE RISKS FOR MY, OR MY CHILS/MINOR’S, PARTICIPATION IN THE SPORT OF TRIATHLON AND THE 2015 ROUNDUP LAKE Triathlon.  Such risks include, but are not limited to death, serious injury, impairment and/or permanent or partial disability, falls, contact with other participants and/or their equipment, effects of weather including heat, cold, and/or humidity, swim venue water conditions, surrounding natural environmental hazards, any hazard associated or posed by other participants, spectators, non-participants, pedestrians ,or volunteers, any hazards associated with equipment for event maintenance, control, organization and/or safety, and other manufactured or environmental hazards in, on, or surrounding the event race course.  I also acknowledge that portions of CHAMP Racing, LLC designed race courses are on PUBLIC ROADS and paths and are subject TO THE RISKS AND DANGERS OF VEHICULAR TRAFFIC and pedestrian traffic.  I further attest that the race course may contain hills and uneven/dangerous surfaces, including but not limited to curbs, gravel, sand, dirt, debris from trees, sewer covers, independent construction equipment, and potholes.  THEREFORE, INCONSIDERATION OF THE ACCETPTANCE OF THIS ENTRY FORM, I FOR MY SELF, MY CHILD/MINOR, MY EXECUTORS, HEIRS, AND ADMINISTRAOTORS, WAIVE, RELEASE, AND FOREVER DISCHARGE FROM ANY AND ALL CLAIMS, AND FURTHER COVENANT AGREE NOT TO SUE FOR SUCH CLAIMS, LOSSES, OR LIABLILITIES, WHICH OCCUR TO ME, OR MY CHILS.MINOR, AS A RESULT OF MY, OR MY CHILD/MINOR’S PARTICIPATION IN THIS EVENT.  SUCH CLAIMS, LOSSES, OR LIABILITIES INCLUDE, BUT ARE NOT LIIMITED TO DEATH, INJURY, PARTIAL OR PERMENANT DISABILITY OR DISMEMBERMENT, PROPERTY LOSS AND/OR DAMAGE, MEDICAL AND HOSPITAL BILLS, ATTORNEY FEES AND COURT LITIGATION EXPENSES, OR THEFT OF ITEMS OF ANY KIND WHICH MAY IN THE FUTURE ARISE OUT OF, OR RESULT FROM, OR RELATE IN ANY CAPACITY TO MY, OR MY CHILD/MINOR’S PARTICIPATION, OR TRAVELING TO AND FROM A CHAMP RACING, LLC directed event.  I further assume the aforementioned risks, liabilities, and dangers associated with swimming, road biking, and running.  BY SIGNING THIS WAIVER/RELEASE FORM, I INDEMNIFY, HOLD HARMLESS, AND AGREE NOT TO SUE OR HOLD NEGLIGENT ANY ENTITIES ASSOCIATED WITH THIS EVENT.  SUCH ENTITIES INCLUDE, BUT ARE NOT LIMITED TO CHAMP Racing, LLC, event sponsors, race directors, volunteers, lifeguard safety patrol, all government, city, and village bodies in which this event or event course is located  I further attest and certify that I, or my child/minor, is/am physically fit and sufficiently prepared and trained for participation in this event, AND I, OR MY CHILD/MINOR, HAVE/HAS NOT BEEN ADVISED AGAINST PARTICIPATING BY A QUALIFIED HEALTH PROFESSIONAL.  I also grant consent to CHAMP Racing, LLC for the use of my, or my child/minor’s, photograph in future promotional literature/web-site information.
