I understand that there are risks associated with strenuous physical exertion with this event, including but not limited to those caused by the terrain, the weather and the condition of the athlete, the vehicular traffic, other participants and lack of hydration. I also understand that if in doubt of any physical condition to engage in the 5K or 1 Mile I have been advised to seek the advice of a competent physician and abide by his/her advice. I certify that I am in good physical condition and capable of participating in this event.  I have been warned I must be in good health to participate in this event.    In consideration of the right to participate in the Wedding Fun Run 5K or 1 mile walk, I hereby waive, release and discharge any and all claims for damages, for death, personal injury or property damage or actions of any kind which may hereafter occur to me or any other persons as a result of my participation in this event. This waiver and release shall be binding on my heirs, my executors and administrators and shall favor Wedding Fun Run, LLC (including the respective affiliates, employees, agents, officers, directors and shareholders of such officials and sponsors), City of Phoenix, Arizona Falls Canal (SRP), Raceplace Events Systems, USAT&F,  and all promoters, coordinating groups, officials, and any other individuals associated with the event, their representatives, successors and assigns and will hold them harmless for any and all injuries or  illness suffered in connection with said event.  You consent to medical care and transportation in order to obtain treatment in the event of injury to you and understand that this Agreement and Waiver extends to any liability arising out of or in any way connected with the medical treatment and transportation provided in the event of any emergency and/or injury.  You understand that no medical care may be available, but if it is, you assume liability for any and all medical expenses incurred as a result of participation in Wedding Fun Run including but not limited to ambulance transport, hospital stays, physician and pharmaceutical goods and services.    IN CONSIDERATION OF THE ACCEPTANCE OF YOUR REGISTRATION AND PARTICIPATION IN THE EVENT, YOU ASSUME FULL AND COMPLETE RISK AND RESPONSIBILITY FOR ANY DISCOMFORT, ILLNESS, INJURY, OR ACCIDENT WHICH MAY OCCUR WHILE YOU ARE PREPARING FOR THE EVENT, DURING THE EVENT, WHILE YOU ARE ON THE PREMISES OF THE EVENT, OR WHILE YOU ARE TRAVELING TO OR FROM THE EVENT. YOU UNDERSTAND THAT PARTICIPATING IN THE EVENT MAY BE HAZARDOUS, AND THAT YOU SHOULD NOT ENTER AND PARTICIPATE UNLESS YOU ARE MEDICALLY ABLE AND PROPERLY TRAINED. YOU SHOULD CONSULT YOUR DOCTOR BEFORE PARTICIPATING IN THE EVENT. IT IS YOUR RESPONSIBILITY TO CHECK AND TO ENSURE THAT YOU ARE AT ALL TIMES MEDICALLY AND PHYSICALLY FIT TO PARTICIPATE IN THE ACTIVITIES RELATED TO THE EVENT. YOU ACKNOWLEDGE AND AGREE THAT THE EVENT MAY BE HELD OVER PUBLIC ROADS AND FACILITIES OPEN TO THE PUBLIC DURING THE EVENT AND UPON WHICH HAZARDS ARE TO BE EXPECTED. YOU ALSO ACKNOWLEDGE AND AGREE THAT PARTICIPATION IN THE EVENT MAY CARRY WITH IT CERTAIN INHERENT RISKS AND DANGERS THAT CANNOT BE ELIMINATED COMPLETELY RANGING FROM RISK OF MINOR DISCOMFORT TO CATASTROPHIC INJURIES INCLUDING PERMANENT DISABILITY AND DEATH. YOU ARE AWARE OF AND ASSUME ALL RISKS ASSOCIATED WITH PARTICIPATING IN THE EVENT, INCLUDING WITHOUT LIMITATION RISKS OF PERMANENT INJURY OR DEATH DUE TO FALLS, OBSTACLES, CONTACT WITH OTHER PARTICIPANTS, ACTS OR OMISSIONS OF OTHER PARTICIPANTS, EFFECT OF WEATHER, TRAFFIC AND CONDITIONS OF ANY ROAD.   You agree to observe and obey all posted rules and warnings, to follow any instructions or directions provided to you by Wedding Fun Run, LLC or the event organizer and to abide by any decision of any event official.    I hereby grant full permission to any and all the above to use my name, my likeness and any photograph, videotapes, motion pictures, recordings or any other record of my participation in this event for any publicity and/or promotional purposes.   No use of rollerblades, roller skates, roller shoes, skateboards, or any roller/motorized object will be allowed as a participant in this event.   No dogs will be permitted at this event.   In filling out this form, I acknowledge I am an amateur in such events.   In filling out this form, I acknowledge I have read and full understand my own liability and do accept the restrictions.   Sanctioned by the AZ Association of USAT&F.   In filling out this form, I acknowledge that none of the previously listed sponsors or companies is responsible for any lost or stolen personal items. Applicable Law; Consent to Jurisdiction; Other Jurisdiction-Specific Provisions:  You agree that the laws of the State of Arizona, United States of America, without regard to the conflict of laws principles thereof, will apply to all matters relating to this Agreement and Waiver. You irrevocably consent that exclusive jurisdiction for any dispute with Active relating to this Agreement and Waiver resides in the courts of Phoenix, Arizona, and you further agree and expressly consent to the exercise of personal jurisdiction in the courts of Phoenix, Arizona in connection with any such dispute including any claim involving Wedding Fun Run, LLC. ACCEPTANCE. BY INDICATING YOUR ACCEPTANCE OF THIS AGREEMENT AND WAIVER, YOU ARE AFFIRMING THAT YOU HAVE READ THIS AGREEMENT AND WAIVER AND FULLY UNDERSTAND ITS TERMS. YOU UNDERSTAND THAT YOU AND ALL REGISTERED PARTIES ARE GIVING UP SUBSTANTIAL RIGHTS, INCLUDING THE RIGHT TO SUE. YOU ACKNOWLEDGE THAT YOU ARE AGREEING TO THIS AGREEMENT AND WAIVER FREELY AND VOLUNTARILY, AND INTEND BY YOUR ACCEPTANCE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW. IF THE PARTICIPANT IS A MINOR OR INCAPACITATED ADULT, YOU CERTIFY THAT YOU ARE THE PARTICIPANT'S PARENT OR GUARDIAN AND AGREE TO THIS WAIVER AND RELEASE FROM LIABILITY ON BEHALF OF THE PARTICIPANT.  
 *The aggregate total per team (of 5 or more runners) will determine the winning team.  Each Bridal Party Team must have five (5) runners minimum to participate to qualify for the Grand Prize Give-a-way.  There is no maximum number of members that can be signed up per team.  Aggregate team scores that will then be calculated to determine the winning team.  Any team with less than five (5) runners to complete the race will be disqualified from winning the Grand Prize free wedding give-a-way.*
