Knowing Waiver, Release of Liability, Assumption of Risk, Agreement Not to Sue, and Signature: In consideration of your accepting my entry, I, intending to be legally bound, do hereby for myself and my heirs, executors, administrators waive and release any and all rights and claims for damages I may accrue against the persons and organizations afﬁliated with the event including but not limited to The Muddy Mutt Run Walk Obstacle, Sally Edwards and The Sally Edwards Company, Mary Meyer and Mary Meyer Life Fitness, The Thomas Family Farm LLC, Topsoils Northwest Inc., TFC Inc., Marvin and Debra Thomas and their staff, all sponsors, volunteers, staff, subcontractors, agents, attorneys, and representatives (hereafter “Releasees”) for any and all injuries that I may suffer while participating in the Muddy Mutt event or any of its allied or accompanying events, or en route to and from the events. I consent to the use of my image in photos, videos and audio recording and ﬁlm, of my participation in all events. I attest that I am physically ﬁt and sufﬁciently trained for this competition, my physical condition veriﬁed by a licensed M.D. during the last 6 months. I understand that this event is or can be a hazardous activity that includes obstacles and dogs. I assume all risks associated with participating knowing that dogs and humans are participating and am giving up my and my dog’s rights to seek any legal action and any and all clams arising out of my participation in the event or being at the premises against any and all Releasees. As part of the waiver, I acknowledge that I have read and understand all of the above. ONLY COMPLETE THE SECTION BELOW IF YOU ARE A PARENT OR GUARDIAN OF A PARTICIPANT UNDER THE AGE OF EIGHTEEN 918) AND AGREE TO EACH AND EVERY TERM OF THIS AGREEMENT. I, the parent or guardian of the named participant give my approval for this child’s participation in this event and understand this Agreement is binding upon me and my child/ward. I assume all risks and hazards incidental to participation in the Muddy Mutt Run Walk Obstacle. I understand that the terms of this waiver apply to myself, spouse, community property, domestic partner, or similar that I may have, as well as my child/ward.
