I wish to participate in the Run Mom Run event, and as a condition of my being allowed to do so I hereby, release and discharge the Catholic Diocese of Kansas City-St. Joseph, its constituent organizations, including but not limited to Visitation Parish, Visitation School and Visitation Church in Kansas City, Missouri, and their officers, agents, and employees from any and all claims for personal injuries or property damage that I may suffer as a result of my participation in the activity described above.
I hereby, warrant and represent that I am physically fit and capable of taking part in such activity.
I agree to abide by the rules and regulations governing the above described activity and to obey any instructions given by the person or persons having supervision and control over the activity.
I hereby, warrant and represent that I am eighteen years of age or over, and upon request will produce satisfactory proof of such fact.

I, the parent/guardian of this participant, request that he/she be allowed to participate in the Run Mom Run event, and hereby give my permission for such participation. Having read this waiver and knowing these facts, and in consideration of your acceptance of my application, I, for myself and anyone entitled to act on my behalf, waive and release the Hope Challenge 5K, all sponsors, their representatives and successor from all claims of liabilities of any kind, including any claims arising our of negligence of aforementioned parties, arising out of my participation in this event. I grant permission to all of the foregoing to use any photographs, motion pictures, recording, or any other record of this event for any legitimate purpose.
